               PLAN YEAR 9/1/2007 TO 8/31/2008

               CRITICAL REMINDERS AND INFORMATION

Specific schedules applicable to the start of new terms will be published concerning distribution of healthcare identification cards.

In order to be in compliance with the underwriting requirements of the carriers, graduate students with eligible academic appointments and others with eligible academic appointments have thirty (30) days from the appointment date to enroll for health insurance.

After the thirty day initial enrollment period, there must be a qualified change in status to enroll or make changes.

Upon completion of the Change form, please note that supporting information from the departmental administrator is required for the determination of approval or denial of an exceptional request.
